After your initial appointment, we offer these three Follow-up packages:

Three Individual Clinic Visits

  • 3 Follow-up Clinic Visits (in person or telehealth) with a doctor and nurses, which include: Bioenergetic Testing, Urinalysis, Iridology/Tongue/Face/Nails analysis, Nutrition Counseling

Six Individual Clinic Visits

  • 6 Follow-up Clinic Visits (in person or telehealth) with a doctor and nurses, which include: Bioenergetic Testing, Urinalysis, Iridology/Tongue/Face/Nails analysis, Nutrition Counseling

TELEHEALTH VISIT

Step 1. Call the clinic at (615) 624-4489 or email us to schedule a telehealth appointment.

Step 2. Print the required forms (found in corresponding email), NP Intake form and PMA form

Step 3. Once the Biological Blueprint™ packet arrives, follow all instructions provided. Be sure to send the New Patient Intake form and PMA form with biological samples.

Step 4. In the self-addressed and pre-stamped envelope, mail complete forms and biological samples to:
